About the Role

We are seeking an experienced nurse to join our Specialist Community Palliative Care CNS team as a Triage Nurse. This is an exciting opportunity to work in a challenging environment, providing effective clinical triage and support to patients and their families.

Key Responsibilities
  • Act as the first point of clinical contact for patients, assessing their needs and prioritizing referrals.
  • Undertake daily triage of new referrals, ensuring a comprehensive understanding of patient needs and providing initial support and treatment.
  • Support the CNS team with daily Palliative Care Virtual Wards and office-based patient administration.
  • Communicate effectively with patients, relatives, and carers, using advanced communication skills as necessary.
Requirements
  • Registered Nurse with significant previous experience in palliative and end-of-life care.
  • Post-registration experience at Band 6 level, with demonstrable experience in prioritizing workloads.
  • Sound knowledge and skills working with IT, including clinical assessment and communication skills.
