About the Role

We are seeking an experienced Registered Nurse to join our Specialist Community Palliative Care CNS team as a Triage Nurse. This is an exciting opportunity to work in a challenging environment, providing effective clinical triage and support to patients and their families.

Key Responsibilities
  • Act as the first point of clinical contact for patients, following referral.
  • Identify and respond to immediate specialist palliative care needs, through direct liaison with patients, carers, and other health and social care professionals.
  • Undertake daily effective and comprehensive triage of all new referrals to the Community Palliative Care CNS team.
  • Carry out holistic initial assessments via telephone, safety netting, and providing initial support and treatment.
  • Support with daily Palliative Care Virtual Wards and offer support to the CNS team in office-based patient administration.
  • Triage messages from patients on the current caseload to offer clinical support, advice, and assessment, prioritising effectively.
