About the Role

We are seeking an experienced and skilled nurse to join our Specialist Community Palliative Care CNS team as a Triage Nurse. This is an exciting opportunity to work in a challenging environment, providing high-quality care to patients and their families.

Main Responsibilities
  • Act as the first point of clinical contact for patients, following referral.
  • Identify and respond to the immediate specialist palliative care needs of patients, through direct liaison with patients, carers, or other involved health and social care professionals.
  • Undertake daily effective and comprehensive triage of all new referrals to the Community Palliative Care CNS team, including contacting the referrer for additional accurate information to ensure a holistic understanding of patient need.
  • Carry out holistic initial assessments via the telephone, safety netting, and providing any initial support and treatment.
  • Support with daily Palliative Care Virtual Wards and offer support to the CNS team in office-based patient administration, such as ordering equipment, providing packages of care, and telephone support to ongoing patients.
  • Triage messages from patients on the current caseload to offer clinical support, advice, and assessment, prioritizing effectively.

Person Specification
  • Registered Nurse with significant previous experience
  • Palliative Care qualification (desirable)
  • Post-registration experience with demonstrable experience within palliative and end-of-life care
  • Experience working at Band 6 level
  • Ability to confidently prioritize workloads
  • Teaching experience (desirable)
  • Clinical assessment skills
  • Sound knowledge and skills working with IT
  • Advanced communication skills
