Palliative Care Triage Nurse Position

An exciting opportunity has arisen for an experienced nurse to join the Specialist Community Palliative Care CNS team as a Triage Nurse.
We are seeking a skilled and motivated nurse to work in a supportive role alongside the CNS team, clinically triaging new patients and urgent patients into the service.
Main Responsibilities- Act as the first point of clinical contact for patients, following patient referral.
- Identify and respond to the immediate specialist palliative care needs of patients, through direct liaison with patients, carers, or other involved health and social care professionals.
- Undertake daily effective and comprehensive triage of all new referrals to the Community Palliative Care CNS team, including contacting the referrer for additional accurate information to ensure a holistic understanding of patient need.
- Carry out holistic initial assessments via the telephone, safety netting, and providing any initial support and treatment.
- Support with daily Palliative Care Virtual Wards and offer support to the CNS team in office-based patient administration, such as ordering equipment, providing packages of care, and telephone support to ongoing patients.
- Triage messages from patients on the current caseload to offer clinical support, advice, and assessment, prioritizing effectively.
