We are seeking an experienced nurse to join our Specialist Community Palliative Care CNS team as a Triage Nurse. This is an exciting opportunity to work in a challenging environment, providing effective clinical triage and support to patients and their families.
Key Responsibilities- Initial Patient Contact: Act as the first point of clinical contact for patients, following referral, and identify immediate specialist palliative care needs.
- Triage and Assessment: Undertake daily effective and comprehensive triage of new referrals, including contacting referrers for additional information, and carry out holistic initial assessments via telephone.
- Support and Treatment: Provide initial support and treatment, safety netting, and offer clinical support, advice, and assessment to patients on the caseload.
- Referral Management: Play a lead role in receiving and managing referrals, identifying appropriateness, urgency, and level of priority, and triage incoming referrals considering patient symptoms.
- Collaboration and Communication: Work collaboratively with internal and external partners to support the coordination and management of patients, and ensure care is planned to meet individualized and dynamic needs.
- Specialist Assessments: Undertake specialist assessments of patients with moderate/complex palliative care needs, providing education for patients, relatives, and carers.
- Discharge and Communication: Receive and manage referrals, discharge patients appropriately, and communicate effectively with patients, relatives, and carers using specialist skills.
