Position Overview

An exciting opportunity has emerged to become a vital part of the specialist services team in an inpatient Social Worker capacity, serving as a Forensic Pathway Coordinator. The Coordinator will play a crucial role within the newly established Secure Pathway Hub of Lancashire and South Cumbria NHS Foundation Trust. This position involves collaborating closely with various agencies and organizations, both internal and external, to ensure effective and efficient inpatient care pathways. The role includes leading discharge planning efforts, identifying potential barriers to discharge early, and taking proactive measures to prevent unnecessary delays. Additionally, the Coordinator will facilitate complex multi-agency discussions regarding discharge processes.

The Secure Mental Health Care services for Lancashire and South Cumbria are provided at a facility that has grown since its inception, currently offering a range of inpatient and community services.

The facility comprises several main service lines, including:

- Men's service

- Women's service

- Acquired Brain Injury Service (Men's)

These services cater to adults in Medium and Low Security, as well as Step Down environments, each with its own care model and tailored training programs in addition to comprehensive induction processes.

Qualifications and Skills
