About the Role

We are seeking an experienced Registered Nurse with a passion for working with individuals with learning disabilities to join our team as a Senior Practitioner CHC. As a key member of our Continuing Healthcare team, you will play a vital role in assessing eligibility for Continuing Healthcare funding, planning and commissioning packages of care, and supporting individuals, their families, and health professionals through the Continuing Healthcare process.

Key Responsibilities
  • Assess eligibility for Continuing Healthcare funding in accordance with the NHS National Framework for Continuing Healthcare and Funded Nursing Care 2009 (revised 2022)
  • Plan and commission packages of care, including Personal Health Budgets, to meet the needs of individuals with complex health needs
  • Support individuals, their families, and health professionals through the Continuing Healthcare process, including transition from child to adult services
  • Review eligibility and packages of care to ensure they meet the needs of individuals with complex health needs
  • Work collaboratively with multi-disciplinary teams, including social workers, psychologists, and other healthcare professionals, to ensure consistent and coordinated care
  • Communicate effectively with service users, their families, and carers to ensure they are fully briefed and understand commissioning and care management processes
Requirements
  • Registered Nurse with a valid NMC registration
  • Experience of working with individuals with learning disabilities and complex health needs
  • Sound understanding of the Continuing Healthcare Framework, including Funded Nursing Care and Section 117 aftercare
  • Knowledge and understanding of Mental Health and Mental Capacity Acts
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Flexibility to work across a large rural area and different services
