About the Role

We are seeking a highly skilled and experienced Senior Mental Health Practitioner to join our team at Devon Partnership NHS Trust. As a key member of our community mental health team, you will be responsible for providing high-quality care and support to older adults with complex mental health needs.

Key Responsibilities
  • Undertake comprehensive assessments of new clients and develop personalized care plans to address their mental health needs.
  • Provide expert clinical guidance and support to colleagues and other healthcare professionals to ensure seamless care coordination.
  • Work collaboratively with multidisciplinary teams to develop and implement effective care pathways and interventions.
  • Contribute to the development and implementation of service improvement initiatives to enhance the quality of care and services.
  • Participate in clinical supervision and peer review to ensure high standards of practice and ongoing professional development.
Requirements
  • Registered Mental Health Nurse (RMN), Social Worker, or Occupational Therapist with a strong background in community mental health.
  • Substantial experience of working with older adults with complex mental health needs, including dementia and functional mental health presentations.
  • Excellent communication and interpersonal skills, with the ability to engage with clients, families, and colleagues from diverse backgrounds.
  • Ability to work independently and as part of a team, with a strong focus on collaboration and partnership working.
  • Strong clinical assessment and problem-solving skills, with the ability to develop and implement effective care plans.
  • Computer literacy and proficiency in electronic record-keeping systems.
