We are seeking a highly skilled and experienced Learning Disability Nurse Practitioner to join our team at Devon Partnership NHS Trust. As a Senior Practitioner - Continuing Healthcare, you will play a crucial role in assessing eligibility for Continuing Healthcare Funding and commissioning packages of care for individuals with complex health needs.
Key Responsibilities- Conduct comprehensive assessments to determine eligibility for Continuing Healthcare Funding in accordance with national framework.
- Commission and procure packages of care that meet the needs of individuals with complex health needs.
- Monitor and review quality of care and support arrangements to ensure individuals are being safely supported in their local communities.
- Support discharge planning from hospital and commissioning of future packages of care and support.
- Lead on brokerage and procurement of alternative care and support for clients whose placements have broken down.
- Support safeguarding investigations by reviewing quality and provision of health-funded packages of care.
- Support transition of young people who are eligible for health-funded packages of care when entering adulthood.
- Registered Nurse with Learning Disability experience.
- Postgraduate qualification relevant to specialism (desirable).
- Ability to be mobile across a geographical area to meet the demands of the role.
- Sound understanding of the Continuing Healthcare Framework, including Funded Nursing Care and Section 117 aftercare.
- Knowledge and understanding of Mental Health and Mental Capacity Acts.
- Skilled communicator with excellent clinical assessment skills.
- Ability to work flexibly within a complex environment and negotiate with complex individuals.
- Time management and prioritization skills.
- Ability to work across a large rural area and different services.
