About the Role

We are seeking a highly skilled and experienced Learning Disability Nurse Practitioner to join our team at Devon Partnership NHS Trust. As a Learning Disability Nurse Practitioner, you will be responsible for assessing eligibility for Continuing Healthcare Funding, planning and commissioning packages of care, and supporting individuals, their families, and health professionals through the Continuing Healthcare process.

Key Responsibilities

  • Assess eligibility for Continuing Healthcare Funding and plan and commission packages of care
  • Support individuals, their families, and health professionals through the Continuing Healthcare process
  • Work collaboratively with multi-disciplinary teams to ensure consistent and well-coordinated care
  • Communicate complex and sensitive information to service users, families, and carers
  • Participate in and chair clinical meetings to agree DSTs and Section 117 reviews
