About the Role

We are seeking an experienced Specialist Community Physiotherapist to join our dynamic and forward-thinking Community team at the Royal Devon University Healthcare NHS Foundation Trust. As a key member of our team, you will be responsible for providing high-quality patient care in patients' homes, nursing and residential homes, and other community settings.

Key Responsibilities

  • Assess and manage patients with a wide range of presentations, including complex neurological and orthopaedic conditions, dementia, and falls prevention.
  • Develop and implement individualized rehabilitation plans to maximize patient independence and quality of life.
  • Work collaboratively with other healthcare professionals, including community nurses and social workers, to provide comprehensive patient care.
  • Provide education and support to patients, carers, and families to promote self-management and independence.
  • Contribute to the development and implementation of community-based services and initiatives to improve patient outcomes.

Requirements

  • Registered Physiotherapist with the Health and Care Professions Council (HCPC).
  • Experience in community rehabilitation and management of complex patient presentations.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, carers, and other healthcare professionals.
  • Ability to work autonomously and as part of a team, with a flexible approach to working hours and locations.
  • Commitment to ongoing professional development and continuing education.
