About the Role

An exciting opportunity has arisen for a skilled Community Nurse to join the Bideford Community Nursing Team at the Royal Devon and Exeter NHS Foundation Trust.

As a Community Nurse, you will play a vital role in providing high-quality care to individuals in their own homes or relevant care settings, promoting and supporting their independence through assessments, implementation, and reviewing of care.

You will be part of a skilled and dedicated team, working under the direct supervision of the Clinical Nurse Specialist, and will be required to be an autonomous practitioner.

The successful candidate will be a pivotal part of the Integrated Health and Social Care team, working collaboratively to provide multidisciplinary working that enhances the care delivered to individuals.

Key responsibilities include:

  • Providing comprehensive assessments and care planning for individuals
  • Implementing and reviewing care plans to ensure high standards of nursing care
  • Working collaboratively with other healthcare professionals to provide seamless care
