Position Overview
Are you an experienced Clinical Practitioner eager to contribute to a community-focused multidisciplinary team? We are seeking a dedicated Senior Clinical Practitioner who is passionate about enhancing patient care and making a significant impact in the community.
The Senior Clinical Practitioner will rotate between our Home Healthcare and Urgent Care Response Teams. The Home Healthcare service allows patients to receive safe and effective treatment in their own homes for a duration of up to 14 days. You will conduct medical evaluations at patients' residences alongside our General Practitioners, with overall management under a hospital consultant. Referrals are received from various community services, emergency services, and healthcare providers, emphasizing the importance of keeping patients safe at home whenever feasible. As a skilled communicator with excellent organizational abilities, you will excel in a multidisciplinary team environment and play a crucial role in developing and supporting this innovative service, ensuring high-quality care for our patients. Your commitment to ongoing professional development and service enhancement will involve participation in projects and trust initiatives. Regular supervision, in-service training, and support will be provided.
Key Responsibilities
As a confident communicator with strong organizational skills, you will thrive in a collaborative team environment and be pivotal in the development and support of this new service.
You will have the opportunity to work across both hospital and community settings, fostering essential connections and networks with acute and community colleagues. This will enable you to utilize your specialized clinical skills across all four pillars of advanced clinical practice to identify the most suitable pathways for your patients, including access to virtual patient monitoring for those requiring additional clinical oversight.
Collaborating closely with the multidisciplinary team across acute, community, and voluntary sectors, you will ensure safe and effective discharges while preventing readmissions.
Drawing on your experience and expertise, you will mentor other team members to guarantee the delivery of high-quality services for our patients, providing education and support as necessary.
We take pride in promoting quality improvement initiatives, service development, and both professional and personal growth. As a member of our team, you will receive regular supervision, in-service training, and support to pursue postgraduate education.
You will engage in team projects and broader trust initiatives, allowing you to build strong professional relationships with a diverse range of multidisciplinary team members.
