Primary Care Mental Health Practitioner

We are seeking a highly skilled and experienced Primary Care Mental Health Practitioner to join our team at the South Westminster Community Mental Health Hub. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality mental health services to adults in our community.
Key Responsibilities- Conduct comprehensive psycho-social mental health assessments to establish care needs and develop effective care plans.
- Act as a liaison between primary care and secondary care services, facilitating referrals and ensuring seamless transitions for patients.
- Provide short-term interventions for clients with complex and severe mental health issues, working collaboratively with other healthcare professionals.
- Support the safe discharge of patients from secondary care, ensuring continuity of care and access to support services.
- Develop and maintain strong relationships with GPs, practice nurses, and other healthcare professionals to ensure effective communication and collaboration.
- Participate in regular supervision and training to maintain and enhance your skills and knowledge.
- Current professional qualification in mental health nursing or occupational therapy.
- Registration with the relevant professional body.
- Post-registration training and CPD in mental health.
- Experience working in multidisciplinary mental health teams, with a focus on acute and severe mental health issues.
- Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups and individuals.
