Primary Care Mental Health Practitioner

We are seeking a highly skilled and experienced Primary Care Mental Health Practitioner to join our team at the South Westminster Community Mental Health Hub. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality mental health services to adults in our community.
Key Responsibilities- Conduct comprehensive psycho-social mental health assessments to establish care needs and develop personalized care plans.
- Act as a liaison between primary care and secondary care services, facilitating referrals and ensuring seamless transitions for patients.
- Provide short-term interventions and support for clients with complex mental health needs, working collaboratively with GPs, practice nurses, and other healthcare professionals.
- Develop and maintain strong relationships with clients, carers, and community partners, promoting a culture of recovery and empowerment.
- Participate in regular supervision and contribute to the development of the team's practice and policies.
- Current professional qualification in mental health nursing or occupational therapy, with registration with the relevant professional body.
- Extensive post-registration experience in mental health, with a strong understanding of psycho-social interventions and risk assessment.
- Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
- Knowledge of current legislation and guidance applicable to mental health services, including the Mental Health Act and the Care Act.
