Key Responsibilities- Conduct comprehensive psycho-social mental health assessments to establish care needs and develop personalized care plans.
- Act as a liaison between primary care and secondary care services, facilitating referrals and ensuring seamless transitions for patients.
- Provide short-term interventions and support for clients with complex mental health needs, working collaboratively with GPs and other healthcare professionals.
- Develop and maintain effective relationships with clients, carers, and other stakeholders, using a person-centered approach to care.
- Contribute to the development and implementation of mental health services, working closely with the Hub Manager and other team members.
- Current professional qualification in mental health nursing or occupational therapy, with registration with the relevant professional body.
- Extensive post-registration experience in mental health, with a strong understanding of psycho-social interventions and risk assessment.
- Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
- Ability to work autonomously and make sound decisions, with a strong commitment to patient-centered care.
