About the Role

We are seeking a highly skilled and experienced Primary Care Mental Health Practitioner to join our team at the South Westminster Community Mental Health Hub. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality mental health services to adults in our community.

Key Responsibilities
  • Conduct comprehensive psycho-social mental health assessments to establish care needs and develop personalized care plans.
  • Act as a liaison between primary care and secondary care services, facilitating referrals and ensuring seamless transitions for patients.
  • Provide brief interventions and support to patients with complex mental health needs, working collaboratively with GPs and other healthcare professionals.
  • Develop and maintain strong relationships with GPs, practice nurses, and other stakeholders to ensure effective communication and coordination of care.
  • Participate in regular supervision and contribute to the development of the team's skills and knowledge.
Requirements
  • Current professional qualification in mental health nursing or occupational therapy.
  • Registration with the relevant professional body.
  • Post-registration experience in mental health, with a focus on primary care and community settings.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups and individuals.
  • Ability to assess, plan, implement, and evaluate comprehensive care packages for patients with complex mental health needs.
