About the Role

We are seeking a highly skilled and experienced Primary Care Mental Health Practitioner to join our team at the South Westminster Community Mental Health Hub. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality mental health services to adults in our community.

Key Responsibilities
  • Conduct comprehensive psycho-social mental health assessments to establish care needs and develop personalized care plans.
  • Act as a liaison between primary care and secondary care services, facilitating referrals and ensuring seamless transitions for patients.
  • Provide short-term interventions and support for clients with complex mental health needs, working collaboratively with GPs, practice nurses, and other healthcare professionals.
  • Develop and maintain effective relationships with clients, carers, and other stakeholders, using a person-centered approach to care.
  • Participate in regular supervision and contribute to the development of the team's practice and policies.
Requirements
  • Current professional qualification in mental health nursing or occupational therapy, with registration with the relevant professional body.
  • Extensive post-registration experience in mental health, with a strong understanding of the needs of diverse client groups.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
  • Ability to assess, plan, implement, and evaluate comprehensive care packages, using a wide range of resources.
