About the Role

We are seeking a highly motivated and experienced Care Team Manager to join our team at Radis Community Care. As a Care Team Manager, you will play a vital role in ensuring the smooth operation of our Supported Living Service in Reading.

Key Responsibilities
  • Work closely with the office team to ensure the service runs efficiently and effectively.
  • Ensure that Support Plans, Risk Assessments, and other paperwork are completed and updated regularly.
  • Support Support Workers with direct care visits and complete monitoring checks, including medication competency and carer assessments.
  • Work with clients to ensure they receive the best possible care and support.

Requirements
  • Minimum 1 year of experience working in the health and social care sector.
  • Experience working in a similar Supported Living or Mental Health setting.
  • Ability to work a range of shift patterns, including alternate weekends.
  • Access to a vehicle for travel between services in the Reading area.
  • Currently hold or working towards a NVQ Level 3.
