Job Summary:
Voyage Care Ltd is seeking a highly skilled and experienced Care Manager Home Care Specialist to join our team. As a key member of our management team, you will be responsible for the day-to-day running of our home care services, ensuring that our clients receive the highest level of care and support.
Key Responsibilities:
- Develop and implement person-centered support plans to meet the unique needs of our clients.
- Conduct thorough care needs assessments to identify individual requirements.
- Recruit, induct, and supervise a team of care professionals to deliver high-quality care services.
- Manage the service's budget and financial performance, ensuring compliance with regulatory requirements.
- Collaborate with local authorities, healthcare professionals, and families to ensure seamless care delivery.
- Lead and participate in multi-disciplinary meetings to discuss client care plans and outcomes.
- Ensure regulatory, legislative, and internal requirements are met, including notifications and adherence to policies and procedures.
Requirements:
- Previous experience as a CQC registered manager or in a care management role.
- Willingness and ability to undertake CQC registration and a Level 5 in Health & Social Care.
- Experience working with individuals with autism, complex mental health needs, traumatic backgrounds, and challenging behaviors.
