We are seeking a highly skilled and experienced Care Manager to join our team at Voyage Care Ltd. As a key member of our management team, you will be responsible for leading our residential service in Woodley/Reading, ensuring that individuals gain more independence in their daily lives and achieve positive outcomes.
The successful candidate will have a passion for enhancing the quality of life of the people we support and will work closely with local authorities, healthcare professionals, and families to deliver high-quality care services.
Key Responsibilities- Manage the day-to-day running of the service(s), developing and implementing person-centered support plans.
- Conduct care needs assessments, recruit and induct new colleagues, complete supervisions/appraisals, and medication audits.
- Attend multi-disciplinary meetings, complete quarterly audits and reports, and effectively manage the service's P&L.
- Ensure regulatory, legislative, internal, and contractual requirements are met, including relevant notifications and adherence to policies and procedures.
- Previous experience as a CQC registered manager or in a care management role.
- Willingness and ability to undertake CQC registration and a Level 5 in Health & Social Care.
- Experience working with individuals with autism, complex mental health needs, traumatic backgrounds, and challenging behaviors.
