In alignment with the NMC Code of Conduct, we are seeking a skilled Clinical Nurse Specialist to become a vital part of our team at Bridge Street Surgery. This role emphasizes the clinical evaluation and management of long-term health conditions, particularly with a concentration on diabetes care.
The successful candidate will adhere to established protocols and guidelines, delivering nursing interventions to patients both autonomously and collaboratively with fellow nursing staff and general practitioners.
The candidate will possess the necessary qualifications and expertise, operating within the framework of patient group directions where applicable.
Key Responsibilities
Overseeing chronic disease management, including diagnosis, monitoring, and the formulation of personalized management strategies. Demonstrating proficiency in diabetes care and respiratory health. Ensuring adherence to treatment plans through a holistic, patient-centered methodology. Evaluating treatment outcomes and suggesting modifications as needed. Conducting assessments of routine wounds, including trauma cases. Performing comprehensive health evaluations for patients. Identifying critical issues and collecting sufficient information for referrals (substance use; domestic abuse; child safeguarding; vulnerable adults; elderly patients; social challenges). Recognizing health concerns and gathering adequate information for referrals (women's and men's health, sexual health, geriatric care). Carrying out monitoring tasks (such as cervical smears) and providing appropriate guidance. Administering vaccinations for travel health and offering advice in accordance with established guidelines. Providing immunizations for both adults and children. Assisting general practitioners with minor procedures as necessary.About Bridge Street Surgery

Qualifications and Experience
Essential
Current registration with the NMC. Qualified Registered General Nurse (RGN). Completion of a relevant post-registration course in diabetes management.Knowledge and Skills
Essential
Proven experience in managing and supporting patients with diabetes, including: Assessing and implementing treatment plans for diabetic patients. Monitoring treatment effectiveness and making necessary adjustments. Providing education on diabetes management, including lifestyle modifications and medication adherence. Evidence of continuous professional development.Desirable
Experience with other chronic conditions such as asthma, COPD, or cervical screening. Familiarity with immunization programs or clinics. Awareness of current trends in General Practice. Understanding of General Practice systems and frameworks such as Quality and Outcomes Framework (QOF) and enhanced services.
