About the Role

We are seeking an experienced Mental Health Practitioner to join our dynamic and friendly Integrated Mental Health Team based in the Midlands region. As a key member of our team, you will provide high-quality care for service users experiencing mental health conditions, working collaboratively with a multidisciplinary team to deliver holistic care and treatment plans.

Key Responsibilities
  1. Complex Caseload Management: Take responsibility for the management of a complex caseload, including specialist assessment, care planning, and evaluation of individual treatment plans.
  2. Quality Standards and Auditing: Participate in the setting of quality standards, including auditing, monitoring, and reviewing practice in line with current clinical guidance and policy.
  3. Medication Administration: Administer medication, complying with relevant Trust procedures and NMC Guidance.
  4. Patient Care Needs Assessment: Assess patient's individual care needs, develop, implement, and evaluate programmes of care.
  5. Interventions and Risk Assessments: Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance, including pharmacological and psychologically informed interventions, and complete and contribute to robust risk assessments and risk management plans.
  6. Positive Risk Taking: Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.

Requirements

The usual hours of work are Monday to Friday 9am – 5pm. You will also be required to have access to a vehicle for work purposes and hold a full licence.
