We are seeking an experienced Mental Health Practitioner to join our dynamic and friendly Integrated Mental Health Team based in the Midlands.
The team provides high-quality care for service users experiencing mental health conditions, and we are looking for a skilled professional to lead on the care and treatment of individuals in their journey towards recovery.
Key Responsibilities- Manage a complex caseload, including specialist assessment, care planning, and evaluation of individual treatment plans.
- Participate in setting quality standards, auditing, monitoring, and reviewing practice in line with current clinical guidance and policy.
- Administer medication, complying with relevant Trust procedures and NMC Guidance.
- Assess patient care needs, develop, implement, and evaluate programs of care.
- Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance.
- Complete and contribute to robust risk assessments and risk management plans.

Person SpecificationQualifications and Training
- A recorded/registered qualification in one of the following: RMN, Social Work, Occupational Therapy.
- Active on NMC/HCPC Register.
Experience
- Experience of working with a client group who present with difficulties relating to emotional dysregulation, high-risk challenging and chaotic behavior, highly complex needs relating to trauma.
- Experience of working with people who have difficulty engaging with services.
