Job Title: Band 6 Community Mental Health Practitioner

We are seeking a highly skilled and dedicated Band 6 Community Mental Health Practitioner to join our dynamic and friendly Integrated Mental Health Team based in Cannock.

The successful candidate will be responsible for managing a complex caseload, including specialist assessment, care planning, and evaluation of individual treatment plans. As part of this role, you will also be expected to:

  • Participate in the setting of quality standards, including auditing, monitoring, and reviewing practice in line with current clinical guidance and policy.
  • Administer medication, complying with relevant Trust procedures and NMC Guidance.
  • Assess patients' individual care needs, develop, implement, and evaluate programmes of care.
  • Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance.
  • Complete and contribute to robust risk assessments and risk management plans.
  • Provide leadership with the MDT, support junior colleagues in their development, and facilitate student nurses in their training.
