Exciting Opportunity for a Band 6 Clinician in a Crisis and Home Treatment Team

We are seeking a highly skilled and experienced Clinician to join our Crisis and Home Treatment Team at Herefordshire and Worcestershire Health and Care NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in providing crisis assessment and home treatment services to individuals aged 17 and over.

About the Role

  • Operate as part of a dynamic and supportive team to provide crisis assessment and home treatment services.
  • Complete comprehensive risk assessments and liaise with various stakeholders to ensure effective care planning.
  • Work flexible hours, including night shifts, to meet the needs of our service users.

Main Responsibilities

  • Provide crisis and home treatment services to individuals with severe and enduring mental illness.
  • Engage patients and carers in recovery and crisis management, promoting a collaborative approach to care.
  • Undertake psycho-social assessments and maintain accurate documentation to inform care planning.

Requirements

  • RMN/Social Worker qualification and evidence of post-qualification training to post-graduate diploma level.
  • Required to train to MAPA Level 1.
  • Three years of post-registration experience, including one year at a senior level, with clinical leadership in a multi-disciplinary team.
