About the Role

We are seeking a highly skilled and dedicated Community Health Nurse to join our team at Leicestershire Partnership NHS Trust. As a key member of our clinical team, you will play a vital role in delivering high-quality care and support to children and their families in the community.

Key Responsibilities:

  • Deliver the Healthy Child Programme in various community settings, including family homes.
  • Work collaboratively with children, young people, and families to promote healthy growth, development, and well-being.
  • Participate in the development and implementation of individualized care plans to meet the unique needs of each family.
  • Provide education and support to families on healthy living, breastfeeding, and other relevant topics.
  • Collaborate with other healthcare professionals to ensure seamless care and support for families.

Requirements:

  • Registered Nurse with a valid UK driving license and access to a car.
  • Experience working with children, young people, and families in a community health setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse groups.
  • Ability to work autonomously and as part of a team, with a flexible approach to working hours.
