Position Overview

Primary Care Careers is looking for a dedicated Pharmaceutical Care Specialist to join our team on a full-time basis. This role is essential in enhancing the quality of patient care across our healthcare facilities.

The ideal candidate will demonstrate a strong commitment to providing exceptional service by conducting comprehensive medication reviews, proactively managing patient health, and offering support to healthcare staff, patients, and their families regarding medication and prescription inquiries.

Key Responsibilities

  • Serve as the primary resource for all medication-related inquiries, delivering expert advice and information on the use of pharmaceuticals.
  • Lead initiatives focused on patient-centered medication optimization and quality enhancement.
  • Conduct structured medication assessments and provide health education to patients through various communication methods, including in-person consultations and telephonic discussions.
  • Assist in the management and monitoring of patients with complex, chronic health conditions.
  • Gather pertinent patient health data during all consultations.
  • Support clinical improvements through systematic audits and the implementation of agreed-upon quality enhancements.
  • Process specialized treatment and discharge documentation where medication optimization or reconciliation is necessary.
  • Oversee the repeat prescription process by reviewing requests and ensuring timely medication reviews.

Candidate Profile

Qualifications

  • A degree in pharmacy and registration with the relevant regulatory body.
  • Post-qualification experience in a pharmacy setting, whether in a hospital, primary care, or community environment.
  • Qualification as an independent prescriber or working towards this credential.
  • Membership in a recognized pharmaceutical society.

Experience

  • Familiarity with a wide range of common acute and chronic conditions encountered in a general medical practice.
  • Proven ability to demonstrate pharmaceutical expertise and knowledge in core areas.
  • Competence in planning, managing, monitoring, advising, and reviewing care plans for patients with long-term conditions.
  • Accountability for delivering professional expertise and direct service provision as an individual.
  • Ability to prioritize effectively when problem-solving and to refer to senior staff or physicians when necessary.
  • Adherence to legal, ethical, professional, and organizational policies and procedures.
  • Support patients in making informed decisions about their prescribed medications and promote adherence in line with established guidelines.
  • A non-judgmental approach with flexibility and adaptability.
  • Self-motivated with excellent communication skills.
