The ideal candidate will demonstrate a strong commitment to delivering exceptional service by conducting thorough clinical medication assessments to proactively manage patient needs. You will provide invaluable support to healthcare staff, patients, and their families regarding medication and prescription inquiries.
Key Responsibilities
- Serve as the primary resource for all medication-related inquiries, offering expert advice and information on pharmaceutical practices.
- Lead initiatives focused on patient-centered medication optimization and quality enhancement.
- Conduct structured medication assessments and health education sessions for patients through various communication methods, including in-person consultations and telephonic interactions.
- Support the management and monitoring of patients with complex, chronic health conditions.
- Gather and document relevant patient health information during all consultations.
- Facilitate improvements in clinical care through audits and the implementation of agreed-upon quality enhancements.
- Process specialized treatment and hospital discharge documentation where medication optimization or reconciliation is necessary.
- Oversee the repeat prescription process by reviewing requests and ensuring timely medication reviews.

Candidate Profile
Qualifications
- A degree in pharmacy and registration with the General Pharmaceutical Council.
- Relevant post-qualification experience in pharmacy, whether in hospital, primary care, or community settings.
- Qualification as an independent prescriber or working towards this designation.
- Membership in the Royal Pharmaceutical Society (RPS).
Experience
- Familiarity with a wide range of common acute and chronic conditions typically encountered in a general medical practice.
- Proven ability to demonstrate pharmaceutical skills and knowledge in core areas.
- Capability to plan, manage, monitor, advise, and review care plans for patients with long-term conditions.
- Accountability for delivering professional expertise and direct service provision.
- Ability to identify priorities in problem-solving and refer to senior professionals or GPs when necessary.
- Adherence to legal, ethical, professional, and organizational policies and procedures.
- Support patients in making informed decisions about prescribed medications and adherence as per established guidelines.
- A non-judgmental attitude with flexibility and adaptability.
- Self-motivated with excellent communication abilities.
