Community Health Nurse

We are seeking a highly motivated and experienced Community Health Nurse to join our team at Procare Health Ltd. As a Community Health Nurse, you will be responsible for leading a local community nursing team, working closely with local acute hospitals, adult social care, mental health teams, GPs, and Practice Nurses to ensure high-quality care for the local population.
Main Responsibilities:- Lead a local community nursing team to provide high-quality care for patients and carers.
- Work closely with local acute hospitals, adult social care, mental health teams, GPs, and Practice Nurses to ensure effective, efficient, safe, and high-quality nursing care.
- Manage and lead a team to ensure effective, efficient, safe, and high-quality nursing care for patients and carers.
- Operate within a Community Coordination Centre, allocating work and triaging referrals.
- Travel to patients' homes frequently throughout the day across the locality and across Guildford and Waverley.
- Registered Level 1 Nurse with current Nursing and Midwifery Council registration.
- Nurse prescriber (V100).
