Job Title: Community Health Nurse Leader

Job Summary:

We are seeking a highly motivated and experienced Community Health Nurse Leader to join our team at Procare Health Ltd. As a Community Health Nurse Leader, you will be responsible for leading a local community nursing team, working closely with various healthcare professionals to ensure high-quality care for the local population.

Main Responsibilities:

  • Lead a local community nursing team to provide high-quality care for patients and carers.
  • Work closely with local acute hospital, adult social care, mental health teams, GPs, and Practice Nurses to ensure effective and efficient care.
  • Manage and allocate work to team members, ensuring that patients receive the best possible care.
  • Develop and implement strategies to improve patient outcomes and satisfaction.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and coordination.

Requirements:

  • Registered Level 1 Nurse with current Nursing and Midwifery Council registration.
  • Nurse prescriber (V100).
  • Strong leadership and management skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
