Job Summary

We are seeking a skilled and experienced Community Health Specialist to join our team in West Waverley. As a key member of our Primary Care Network, you will play a vital role in ensuring effective communication and collaboration across our network.

Key Responsibilities:
  • Communication and Collaboration: Work collaboratively with GPs, practice nurses, community nurses, social services, and other healthcare professionals to ensure good working relationships and effective communication.
  • Advocacy and Patient Care: Act as an advocate for individuals and carers, prioritizing their needs, wishes, and preferences to ensure a culture of person-centered care.
  • Documentation and Record-Keeping: Maintain accurate and legible documentation and clinical records in line with professional and organisational requirements, including paper records and electronic clinical record systems.
Requirements:
  • Strong Communication Skills: Effective communication and interpersonal skills, with the ability to communicate complex information to patients, families, and carers.
  • Collaboration and Teamwork: Ability to work collaboratively as part of a team, with a focus on building strong relationships and effective communication.
  • Professional Qualifications: Relevant professional qualifications and experience in a healthcare setting.
