About the Role
We are seeking a highly motivated District Nurse with the aptitude, motivation, and leadership skills, to drive local changes and integration within community nursing. As a named District Nurse for a GP practice, accountable for a caseload of patients, you will operationally manage and lead a team to ensure effective, efficient, safe, and high-quality nursing care for patients and carers.
Key Responsibilities
- Lead a local community nursing team, working closely with the local acute hospital, adult social care, mental health teams, GPs, and Practice Nurses to ensure high-quality care for the local population.
- Manage and allocate work to team members through a Community Coordination Centre.
- Ensure effective, efficient, safe, and high-quality nursing care for patients and carers.
- Travel to patients' homes frequently throughout the day across the locality and across Guildford and Waverley.
Requirements
- Registered Level 1 Nurse with current Nursing and Midwifery Council registration.
- District Nursing Specialist Practice Qualification / diploma / degree.
- Assessor / NMC Supervisor / Mentorship.
- Nurse prescriber (V100).
