District Nurse Leader

We are seeking a highly motivated District Nurse with the aptitude, motivation, and leadership skills, to drive local changes and integration within community nursing. As a named District Nurse for a GP practice, accountable for a caseload of patients, you will operationally manage and lead a team to ensure effective, efficient, safe, and high-quality nursing care for patients and carers.
Main DutiesAs a District Nurse, you will be responsible for:
- Leading a local community nursing team, working closely with the local acute hospital, adult social care, mental health teams, GPs, and Practice Nurses to ensure high-quality care for the local population.
- Managing and leading a team to ensure effective, efficient, safe, and high-quality nursing care for patients and carers.
- Working within a Community Coordination Centre, which will allocate work and triage referrals.
- Traveling to patients' homes frequently throughout the day across the locality and across Guildford and Waverley.
