Job summary

You will lead a local community nursing team, working closely with the local acute hospital, adult social care, mental health teams, GPs, and Practice Nurses to ensure high quality care for the local population.

From 1st July 2020 Procare Community Services are providing adult community nursing services through a partnership venture with The Royal Surrey NHS Foundation Trust.

All community nurses use EMIS as the electronic clinical record system and are provided with a work Ipad and Iphone. We have full-time, part time and bank posts available within our adult community nursing teams.

Main duties of the job

We are seeking a highly motivated District Nurse with the aptitude, motivation, and leadership skills, to drive local changes and integration within community nursing. As a named District Nurse for a GP practice, accountable for a caseload of patients, you will operationally manage and lead a team to ensure effective, efficient, safe, and high-quality nursing care for patients and carers.

All work allocation and triage of referrals is through a Community Coordination Centre, which as a District Nurse you will be required to work within. Post holders will be available to work Monday to Sunday covering shifts between 8 am and 8 pm and need to be able to travel to patients homes frequently throughout the day across the locality and across Guildford and Waverley.
