Job Summary

We are seeking a highly motivated District Nurse to lead our community nursing team. As a named District Nurse for a GP practice, you will be accountable for a caseload of patients and operationally manage and lead a team to ensure effective, efficient, safe, and high-quality nursing care for patients and carers.

Main Responsibilities

As a District Nurse, you will:

  • Provide high-quality patient care and lead a team to achieve excellent care outcomes.
  • Manage and allocate work to team members, ensuring efficient and effective use of resources.
  • Work closely with local acute hospital, adult social care, mental health teams, GPs, and Practice Nurses to ensure seamless care coordination.
  • Develop and implement care plans that meet the needs of patients and carers.
  • Collaborate with the Community Coordination Centre to triage referrals and allocate work to team members.

Requirements

To be successful in this role, you will need:

  • Registered Level 1 Nurse with current Nursing and Midwifery Council registration.
  • District Nursing Specialist Practice Qualification / diploma / degree.
  • Assessor / NMC Supervisor / Mentorship.
  • Nurse prescriber (V100).
