Job Summary: We are seeking a highly skilled and compassionate Children's Complex Care Nurse to join our team at Thornbury Community Services. As a Children's Complex Care Nurse, you will be responsible for delivering bespoke care and support to young people with complex needs, enabling them to stabilize at home and achieve their personal goals.

About the Role: This is a challenging yet rewarding role that requires a high level of expertise and experience in caring for children with complex medical conditions. You will work closely with our multidisciplinary team to develop and implement individualized care plans that meet the unique needs of each client.

Key Responsibilities:

  • Deliver high-quality, person-centered care to children with complex medical conditions
  • Develop and implement individualized care plans in collaboration with the multidisciplinary team
  • Provide education and support to clients and their families to promote independence and self-management
  • Work closely with other healthcare professionals to ensure seamless care coordination
  • Contribute to the development and implementation of new care initiatives and programs

Requirements: To be successful in this role, you will need to have a minimum of 12 months of experience working in a similar capacity, with a strong background in paediatric nursing and complex care. You must be fully vaccinated against COVID-19 and comfortable working in a fast-paced, dynamic environment.
