Key Responsibilities- Conduct initial assessments in the community to identify client needs and develop personalized care plans.
- Provide clinical oversight to a team of non-clinical case managers, ensuring high-quality care and support.
- Collaborate with the wider multidisciplinary team (MDT) to coordinate care and services.
- Attend meetings and make necessary referrals to ensure seamless care delivery.
- Manage a team of non-clinical case managers, providing guidance and support to ensure excellent client outcomes.
- RGN qualification with excellent clinical skills in complex care.
- Experience with peg feeds, ventilators, and tracheostomy management.
- A valid driving license, as the role involves remote and community-based work.
- Ability to work independently and as part of a team.
- Annual salary of £48,000.
- Bonus opportunities.
- Full-time, permanent position.
- Monday to Friday, 9am-5pm with some flexibility.
- Two days home-based, three days community visits.
