Job Summary:

Nurse Seekers is excited to be working with a leading specialist care provider to find an experienced Registered Nurse RGN for a challenging and rewarding role in community care.

About the Role:

We are seeking a qualified and experienced Registered Nurse RGN to join our client's team as a Case Manager in Nottinghamshire. The successful candidate will have a strong background in working in a community setting and experience in developing care plans.

Key Responsibilities:

  • Manage a client case load based in and around Derby to provide ongoing visits to create care and support plans
  • Undertake annual support reviews, risk assessments, inductions, supervisions, staff training, and other visits necessary to keep the person safe
  • Develop and implement care plans that promote independence and enable clients to live their life to the fullest

Requirements:

  • Qualified RGN with a strong background in working in a community setting
  • Experience in developing care plans and managing client case loads
  • Full UK Driving Licence and car owner
  • Care planning experience and strong clinical skills
