As a Children's Complex Care Nurse, you will be responsible for delivering bespoke care and support to young people with complex needs, enabling them to stabilize at home and achieve their personal goals.
About the Role: This is a challenging yet rewarding role that requires a high level of expertise and experience in caring for children with complex medical conditions. You will work closely with our multidisciplinary team to develop and implement individualized care plans that meet the unique needs of each client.
Key Responsibilities:
- Deliver high-quality, person-centered care to children with complex medical conditions
- Develop and implement individualized care plans in collaboration with the multidisciplinary team
- Provide education and support to clients and their families to promote independence and self-management
- Collaborate with healthcare professionals to ensure seamless care transitions and coordination
- Participate in quality improvement initiatives to enhance the quality of care and services
Requirements: To be successful in this role, you will need to have a minimum of 12 months of experience working with children with complex medical conditions. You must be a registered nurse with a valid UK nursing license and have a strong understanding of child development and family-centered care. You will also need to be fully vaccinated against COVID-19 and have a valid DBS check.
