About the Role

We are seeking a highly organized and methodical individual to join our team as a Care Coordinator. As a key member of our office team, you will be responsible for coordinating and scheduling the delivery of care for our clients while providing exceptional service.

Key Responsibilities

  • Develop and maintain effective and efficient schedules for our clients and care professionals.
  • Ensure schedules are prepared, considering travel time, holidays, training, and last-minute cancellations.
  • Manage the block scheduling system, conducting quarterly reviews to ensure optimal performance.
  • Respond promptly to changes in the schedule and collaborate with relevant team members.
  • Match care professionals with new clients, working closely with the client services team to arrange introductions.
  • Ensure client schedules are aligned with their needs, with the same care professional and times each week, where possible.
  • Build strong relationships with clients and care professionals to ensure positive experiences.
  • Work with the recruitment team to ensure sufficient current and future staffing levels are met.
  • Collaborate with the care team to ensure new and existing care packages can be resourced and scheduled on a timely basis.
  • Maintain close communication with the client services team to ensure care professionals' ongoing needs and availability are up-to-date.
  • Manage care professionals' annual leave system in accordance with company policies.
  • Participate in on-call duties, responsible for handling out-of-hours queries – one night during the week and one in every six weekends on average.

Requirements

  • Experience in a scheduling role within a home care or relevant environment, such as logistics.
  • Good working knowledge of IT systems, including Microsoft Office or Google Suite, and CRM software, with the ability to learn and adapt new technologies as needed.
  • Highly resilient and positive individual with excellent communication skills.
  • Excellent organizational and prioritization skills, with the ability to work calmly and accurately under pressure.
  • Team player with strong interpersonal skills, able to build rapport quickly.
  • Excellent attention to detail, with the ability to multitask.
  • Logical and analytical, with the ability to work independently and meet deadlines.
