Pharmaceutical Care Coordinator

Position Overview
Care Plus Group is seeking dedicated Pharmaceutical Care Coordinators to enhance our team within the Community Inpatient Unit, focusing on the management of medications for our patients.
Our Community Inpatient Unit serves as a supportive environment for individuals recovering from various health challenges, including illness, trauma, or surgical procedures. We aim to facilitate hospital discharges, prevent unnecessary admissions, and promote independent living through a collaborative approach involving a diverse team of healthcare professionals.
Key Responsibilities
As a Pharmaceutical Care Coordinator, your primary responsibilities will include:
- Administering and overseeing medication management within the unit.
- Collaborating with a multidisciplinary team to ensure optimal patient care.
- Providing guidance and support to patients regarding their medication regimens.
- Maintaining accurate records and documentation related to medication administration.
Qualifications
To be considered for this role, candidates must possess:
- NVQ/QCF Level 3 in Health and Social Care.
- Certification in Safe Handling of Medications or an equivalent qualification.
- Competency in English, Mathematics, and IT at Level 3 or equivalent.
Experience
Essential experience includes:
- Recent involvement in supporting individuals in a healthcare or social care setting.
- Demonstrated knowledge of person-centered care principles.
- Proven experience in safe medication handling.
- Familiarity with IT systems relevant to healthcare.
Skills and Competencies
Successful candidates will exhibit:
- Strong communication skills, both verbal and written.
- Ability to provide emotional and practical support to patients.
- Commitment to maintaining confidentiality and adhering to relevant legislation.
- Understanding of the Mental Capacity Act and its implications for medication management.
- Knowledge of the Care Quality Commission standards and their relevance to medication processes.
- Capability to mentor and support colleagues in medication-related tasks.
Work Environment
This role requires flexibility to work according to a rota, which may include evenings, weekends, and public holidays.
