About the Role

We are seeking a highly skilled and compassionate Assertive Outreach Nurse to join our team at Change Grow Live. As an Assertive Outreach Nurse, you will play a vital role in providing specialist clinical interventions to individuals who are homeless or at risk of homelessness.

Key Responsibilities
  • Conduct comprehensive physical and psycho-social assessments for individuals who are homeless or at risk of homelessness.
  • Develop and implement individualized care plans to address the unique needs of each client.
  • Provide screening for blood-borne viruses (BBV) and offer Hepatitis A and B vaccinations as indicated.
  • Work in partnership with other community teams and voluntary organizations to ensure seamless delivery of care to the homeless population.
  • Act as an advocate for individuals experiencing homelessness, supporting their choices and promoting independence and self-care.
About You

We are looking for a qualified and experienced nurse who is passionate about working with vulnerable populations. You will have a strong understanding of harm reduction and health promotion interventions, as well as the ability to assess and recovery plan service users.
