Job Title: Assertive Outreach Nurse

Join Change Grow Live, a charity dedicated to making a difference in the lives of our service users. We are seeking an experienced Assertive Outreach Nurse to join our team in Southampton.

About the Role:

We are looking for a skilled and compassionate nurse to provide specialist clinical interventions to individuals who are homeless or at risk of homelessness. As an Assertive Outreach Nurse, you will work as part of a multi-disciplinary team to offer support and treatment to our service users.

Key Responsibilities:
  • Conduct physical and psycho-social assessments for the homeless population.
  • Provide clinical and therapeutic treatments based on best practice to improve health outcomes.
  • Screen for blood-borne viruses and provide vaccinations as necessary.
  • Work with other community teams and organizations to ensure seamless delivery of care.
  • Act as an advocate for individuals experiencing homelessness, promoting independence and self-care.
  • Contribute to the CGL Naloxone Strategy by training staff and individuals in the use of naloxone.
  • Offer wound care assessment and basic wound dressing, and signpost to other services as required.
About You:

We are looking for a nurse with recent relevant post-qualification experience and a current PIN. You should have a sound understanding of harm reduction and health promotion interventions, and be able to assess and recovery plan service users. You will also have clinical auditing skills to inform evaluation of the service and identify areas for development.
