We are seeking a highly skilled and motivated Community Diabetes Nurse to join our Integrated Diabetes Nursing service in the borough of Barking and Dagenham and the wider BHR teams.
Key Responsibilities- Facilitate patient structured education and provide high-quality, evidence-based diabetes clinical practice throughout the local community, in health centre clinics, and patients' homes.
- Work closely with the lead Diabetes consultant, GPs, nursing colleagues, and other members of the multidisciplinary team to deliver comprehensive care to patients with diabetes.
- Develop and implement care plans for people with diabetes, utilizing comprehensive and extensive community nursing skills.
- Deliver structured education and presentations to patients and healthcare professionals, utilizing excellent communication skills and presentation skills.
- Work autonomously and manage and prioritize own workload effectively, demonstrating flexibility and adaptability as a team player.
- Registered Nurse on Part 1 of the NMC.
- Minimum of 1 year experience in a Diabetes setting.
- 2 years post-reg experience and working as a multidisciplinary team.
- Flexible and adaptable team player with excellent communication skills.
