About the Role

We are seeking an experienced and motivated Registered Nurse with a Specialist Practitioner qualification to join our Integrated Care Team as a District Nurse Team Leader.

Key Responsibilities
  • Lead a team of Community Nurses and Health Care Support Workers, providing guidance and support to ensure high-quality patient care.
  • Assess, manage, plan, and deliver care to a caseload of patients, ensuring professional and legal accountability.
  • Supervise and appraise staff members and students, promoting their development and growth.
  • Collaborate with peers and locality leads to achieve team goals and objectives.
  • Support the development of the team, identifying areas for improvement and implementing changes to enhance patient care.
Requirements
  • Registered Nurse with a Specialist Practitioner qualification.
  • Experience working in a community setting, preferably with a caseload of patients.
  • Excellent leadership and communication skills, with the ability to motivate and inspire team members.
  • Ability to travel between sites, with a valid UK driving license.
  • Non-medical or community nursing prescribing skills.
