About the Role

We are seeking a highly skilled and motivated Community Diabetes Nurse to join our Integrated Diabetes Nursing service in the borough of Barking and Dagenham and the wider BHR teams.

Key Responsibilities
  • Facilitate patient structured education and provide high-quality, evidence-based diabetes clinical practice throughout the local community, in health centre clinics, and patients' homes.
  • Work closely with the lead Diabetes consultant, GPs, nursing colleagues, and other members of the multidisciplinary team to deliver comprehensive care to patients with diabetes.
  • Develop and implement care plans for people with diabetes, utilizing comprehensive and extensive community nursing skills.
  • Deliver structured education and presentations to patients and healthcare professionals, utilizing excellent communication skills and presentation skills.
  • Work autonomously and manage and prioritize own workload effectively, utilizing flexible and adaptable team player skills.
Requirements
  • Registered Nurse on Part 1 of the NMC.
  • Minimum of 1 year experience in a Diabetes setting.
  • 2 years post-reg experience.
  • Working as a multidisciplinary team.
  • Flexible and adaptable team player.
  • Demonstrate comprehensive and extensive community nursing skills.
  • Proven clinical skills in undertaking assessment, developing, implementing, and evaluating care plans for people with diabetes.
  • Presentation skills and experience of delivering structured education.
  • Excellent communication skills.
  • Ability to work autonomously and manage and prioritize own workload effectively.
