Job Overview

As a Care Coordinator at Home Instead, you will play a pivotal role in ensuring the smooth operation of our care services. Your organizational skills and attention to detail will be essential in managing client care and supporting our dedicated team of professionals.

Key Responsibilities:
  • Input and manage new client inquiries within our system.
  • Schedule care consultations for the Registered Care Manager (RCM).
  • Oversee all necessary arrangements to initiate new services.
  • Collaborate with the RCM to develop effective schedules that guarantee high-quality support for our clients.
  • Work closely with the Recruitment Officer (RO) to identify capacity needs and recruitment opportunities.
  • Ensure that schedules account for travel time, holidays, training, and staff development.
  • Conduct quarterly reviews of the block scheduling system.
  • Respond promptly to scheduling changes and communicate effectively with clients, their families, and team members.
  • Assist in training new team members by completing initial solo calls.
  • Support the RCM in creating, updating, and auditing care plans, needs assessments, e-Mar charts, and visit notes.
  • Manage waiting lists and requests for additional hours from clients.
  • Foster positive relationships with clients and care professionals to enhance their service experience.
  • Coordinate holiday and absence management for the care professional team, ensuring service quality is maintained.
  • Maintain compliant systems, ensuring all records and databases are current.
  • Provide coverage for unallocated visits, holidays, and absences.
  • Handle daily queries from clients and care professionals in accordance with our responsive processes.
  • Perform any other duties essential for the effective operation of the business.
  • Participate in the on-call out-of-hours rota.
Essential Criteria:
  • Proven experience in an administrative planning role.
  • A warm and approachable demeanor.
  • Exceptional administrative skills with the ability to remain calm and accurate under pressure.
  • A strong team player with excellent interpersonal skills, capable of building rapport quickly.
  • Proficient in Microsoft Office 365 or Google Suite, Excel, and CRM software, with a quick adaptability to new technologies.
  • Highly resilient and positive, with outstanding communication skills.
  • Meticulous attention to detail and the ability to manage multiple tasks effectively.
  • A logical and analytical thinker who can work intuitively to achieve objectives.
  • A creative problem-solver focused on delivering high-quality service.
Qualifications

Ideally, candidates should possess an SVQ level 3 in Health & Social Care or be willing to work towards this qualification.
