Position Overview

Are you dedicated to delivering individualized care? We are pleased to present a unique opportunity for a support professional to become part of our Clinical Nurse Specialist Team as a Cancer Support Coordinator. In collaboration with Macmillan, we have established a cancer support coordination service. Your role will involve assisting the Clinical Nurse Specialist in offering personalized care, serving as a primary contact for patients, directing them to additional resources, providing essential information, and conducting comprehensive needs assessments.

We are looking for a skilled Health Care Support Worker who possesses outstanding communication, problem-solving, and organizational abilities.

The ability to communicate in Welsh is preferred for this position; candidates who speak Welsh and/or English are encouraged to apply.

Key Responsibilities

  • Enhance communication skills to effectively relay complex and sensitive information to patients, caregivers, and other team members, while overcoming any barriers to understanding and providing support during challenging or emotional situations.
  • Engage with individuals in a manner that aligns with their understanding and cultural background, recognizing obstacles to effective communication.
  • Maintain clear, effective, and appropriate communication with the multidisciplinary team, patients, and their families or caregivers.
  • Address inquiries appropriately, take phone messages, and relay written and verbal information to patients.
  • Provide pertinent information to patients, families, caregivers, and colleagues.
  • Accurately complete documentation and contribute to reports regarding patient activities and progress, acknowledging that the Clinical Nurse Specialist holds personal accountability for entries made by others under their supervision.
  • Offer feedback on patient care and relevant communications to appropriate healthcare professionals.
  • Participate in discussions regarding patient care and clinical service enhancement, including relevant team meetings.
  • Uphold patient confidentiality at all times.
  • Demonstrate support worker experience.
  • Possess NVQ Level 4 or 3 along with additional training equivalent to diploma level or relevant experience.
  • Have knowledge of cancer-related issues.
  • Exhibit experience in teamwork.

Qualifications and Knowledge

Essential

  • GCSE English Language and Mathematics (Grades A-C) or equivalent.
  • NVQ Level 4 or 3 with additional training equivalent to diploma level or relevant experience.
  • Understanding of patient-centered care principles.
  • Familiarity with basic medical terminology.
  • Ability and willingness to manage highly confidential information while maintaining confidentiality and trust.

Desirable

  • Training in communication skills.
  • Knowledge of cancer and its treatments.

Experience

Essential

  • Relevant health or social care experience at Band 3 or equivalent.
  • Experience in multi-professional collaboration.
  • Effective communication with patients and caregivers using various communication methods.
  • Proficiency in IT systems.
