Key Responsibilities of the Support Services Facilitator include:

• Assisting clients with various learning and physical disabilities to enhance their independence and potential
• Providing individualized support tailored to client needs

• Ensuring safety through regular risk assessments and evaluations of client requirements

• Continuously reviewing and updating support plans based on client assessments

• Building professional relationships with clients, maintaining their dignity, privacy, and personal choices

• Facilitating small group activities aimed at promoting recovery and well-being

Essential Skills and Experience:

• Prior experience working with adults facing physical or learning challenges
• Strong interpersonal skills to connect and empathize with individuals with disabilities

• Ability to perform effectively under pressure

• Proficiency in maintaining accurate records and support documentation

• Basic understanding of Microsoft Office and online communication tools

• NVQ Level 3 in Health and Social Care or a willingness to pursue this qualification
